My best practice is to install Cognos 10 on a new server (or a new VM) with new content store and audit log databases.
Next, run a full content store export on Cognos 8.4 and import the 8.4 export file on your new Cognos 10 installation.
The final step is to test the new installation and modify reports and models when necessary.

We have successfully upgraded Cognos from 8.4 to 10.1.1 few months back.

-> Install Cognos in different server.
-> Clone Content DB of 8.4
-> point 10 instance to the new cloned DB and start services.

rest of the process will be taken care of by Cognos.

Note: If have used lot of customization with HTML and Java Scripts they might break due to incompatibility.

If you need any assistance please let me know.
